Findings
According to the documentation, Alleged Perpetrator (AP2) failed to protect the Alleged Victim (AV) from physical and verbal abuse. The failure resulted in the AV receiving bruises resembling finger and thumb prints on their arm and witnesses observed AP2 yelling at AV resulting in the AV to be distressed and crying, which is a violation of resident rights and is considered physical and verbal abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
