Findings
The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls and change of condition. AV was found on the floor from a fall, which may have been caused by the fall mat slipping. The facility failed to properly care plan for AV's risk of falls, placing AV at risk for serious harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
