Findings
The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, on or about July 14, 2023, AV started showing signs of pain with transfers, unable to bear any weight on his/her right leg and began to refuse care. On or about July 17, 2023, AV had a bruise and significant swelling on his/her thigh and continued to show signs of discomfort with transferring. AV was sent to the hospital on or about July 20, 2024, where he/she was diagnosed with a hip fracture. The facility failed to assure timely medical treatment when AV experienced a significant change of condition, which is a violation of resident rights, is neglect of care which constitutes abuse.
