Findings
The facility failed to follow the Alleged Victim's (AV) care plan to escort him/her to another area when agitated. The failure resulted in a physical altercation where Witness 1 hit AV on the top of his/her head causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
