- Alleged Victim (AV) relies on the facility for his/her care. On or about July 26, 2023, AV was found face down on the floor, blood was observed dripping from a wound that occurred the day before. AV was incoherent and was transported to the ER. AV had a prior fall on July 25, 2023, the fall occurred 10 hours prior to the current fall from July 26, 2023. The facility failed to provide a safe environment for AV by failing provide proper supervision and service to AV, resulting in falls and injuries to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
