Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about July 31, 2023, Alleged Perpetrator 2 (AP2) was the assigned caregiver for AV, AP2 left AV in AV’s wheelchair all night. AV was found with dried feces and redness on his/her buttocks after AP2’s shift. Due to the dried feces, AV required an emergency shower. The dried feces was difficult to get off AV and AV was worn out after receiving a shower to remove the dried feces. After AP2’s shift, AV’s legs were found to be swollen. AP2 failed to provide service to AV, which is a neglect of care and constitutes abuse. The facility failed to assure AV received service, which is a violation of Oregon Administrative Rules.
