Findings
The facility failed to provide appropriate services, training, and staffing levels to meet the Alleged Victim's needs. The failure resulted in AV not receiving appropriate incontinence care on multiple occasions, causing unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse under OAR 411-020-0002(1)(b)(A)(i).
