- Alleged Victim (AV) relies on the facility for his/her care. On or about August 4, 2023, AV passed away and it was discovered that AV had dry, cracked skin that was dirty and falling off in pieces shortly after AV’s passing. AV also had sores going up and down his/her legs. AV’s physician was not aware that AV had dry skin. It was also known that AV would refuse lotion care and for about two weeks and a half, AV did not accept lotion assistance but once. The facility failed to care plan around AV’s refusal to lotion care, leading to his/her poor dry and cracked skin condition, which is a violation of resident rights, is neglect of care and constitutes abuse.
