Findings
The facility failed to implement interventions and appropriately care plan regarding the Alleged Victim’s (AV) known fall risk. AV suffered a fall on August 21, 2023, resulting in a broken hip requiring AV to go to the hospital and then a rehabilitation facility. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
