Findings
Alleged Victim (AV) is service plan to get one (1) shower a week. AV has been requesting showers but due to the facility not having a proper Hoyer to hold AV’s weight, care staff are unable to provide safe showers. The facility failed to follow the care plan around AV’s shower needs, which is a violation of resident rights, is neglect of care and constitutes abuse.
