Findings
Alleged Victim (AV) and Witness (W1) have a history of altercation resulting in injury. On or about September 4, 2023, W1 struck AV with flowers while AV was seated at a table. As a result, AV fell and suffered a skin tear to his/her right forearm. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
