- On or about September 1, 2023, Alleged Perpetrator 2 (AP2) neglected Alleged Victim (AV) by failing to provide services necessary to maintain safety of AV when AP2 gave AV a cold and rough bed bath. Based on facility documentation and interviews, AV was in bed due to a broken hip and as a result of the rough treatment by AP2 as he/she assisted AV with his/her Activities of Daily Living, AV experienced pain and unreasonable discomfort. AP2 was observed pulling, pushing and scrubbing AV in a rough manner during the bed bath. AP2's action is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
