The Alleged Victim (AV) requires a two person assist for transfers. Alleged Perpetrator #2 (AP2) attempted a single person transfer with AV, in which AP2 fell next to or on top of AV, causing a risk of harm. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
