Findings
The facility failed to implement appropriate interventions and supervision according to the Alleged Victim's (AV) needs. On or about September 4, 2023, AV experienced a fall at some point in the night and went approximately 8 hours without staff assistance, was admitted to the hospital for approximately 7 days, and required on-going licensed professional intervention and care. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
