Findings
The facility failed to monitor and provide appropriate supervision related to the Alleged Victim’s (AV) known history of falls. On or about June 11, 2023, AV experienced a fall that resulted in a skin tear, swelling, and bleeding to his/her forehead causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
