Findings
The facility failed to provide appropriate services according to Alleged Victim's (AV) care needs. Based on facility documentation and interviews, the facility failed to assist AV with showers for eight days resulting in AV suffering unreasonable discomfort and loss of personal dignity. The facility's failure to follow AV's care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
