Findings
Witness #1 (W1) is known to have inappropriate behaviors with other residents. On multiple occasions, W1 was found to have been sexually aggressive with the Alleged Victim (AV) causing AV to become upset. The facility's failure to ensure proper care planning for W1's behavior is a violation of resident rights, is considered neglect of care and constitutes abuse.
