Findings
The Alleged Victim (AV) was found on the floor in another residents room on September 10, 2023. AV was seen tripping and falling onto the ground in front of a dresser in the room. There is an area on the floor that is in disrepair and sticking up in front of the dresser, which AV may or may not have tripped on. The facility's failure to provide a safe environment, due to the floor being in disrepair, is a violation of Oregon Administrative Rules.
