Findings
On or about September 13, 2023, Alleged Perpetrator 2 (AP2) roughly transferred Alleged Victim (AV) which caused pain and unreasonable discomfort to AV. AP2 is responsible for physical abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
