Findings
The facility failed to implement interventions or appropriately care plan for Witness #1’s (W1) known behaviors. This failure resulted in W1 hitting or elbowing the Alleged Victim (AV) on or about September 15, 2023, causing unreasonable discomfort to the AV. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
