Findings
The facility failed to implement interventions and appropriately care plan related to AV's dietary needs and risk of choking. The failure to ensure staff were aware, instructed and demonstrated competency to meet AV's needs placed AV at risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
