Findings
The facility and AP2 failed to appropriately respond to the Alleged Victim's (AV) change of condition and provide consistent skin assessments and put in interventions around AV's skin breakdown. The failure resulted in AV requiring on-going licensed professional intervention and treatment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
