Findings
According to the documentation, the facility failed to provide a safe medication administration system to ensure medication did not go missing. The facility did not have an accurate narcotic log which lead to the Alleged Victim (AV) to lose approximately 3.5ml of narcotic medication, which is a violation of Oregon Administrative Rules.
