Findings
The facility failed to implement interventions and/or follow the care plan for Witness #1’s (W1) known behaviors. On or about September 22, 2023, W2 punched the Alleged Victim (AV) in the face, causing unreasonable discomfort and emotional distress. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
