Findings
The facility failed to implement interventions or appropriately care plan for Witness #1’s (W1) and the Alleged Victim's (AV) known behaviors. On or about September 24, 2023, W1 pushed the AV to the ground, resulting in AV having a head injury and going to the hospital. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
