- Alleged Victim’s (AV’s) care plan states he/she is confused at times and is at risk for elopement and will exit seek. On or about September 24, 2023, AV was observed walking outside with no staff member present. AV exited out the side door and no alarm went off; AV typically goes for walks with staff and exits the side door. The facility failed to provide a safe environment for AV, when the alarm did not go off when AV exited the secured building, putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
