Findings
On or about September 25, 2023, Alleged Victim (AV) did not receive his/her correct medication according to the dosage, route, frequency, and time as described on AV's medical order. The facility's failure to provide a safe medication administration system is a violation of Oregon Administrative Rules. Alleged Perpetrator 2 (AP2) allegedly neglected AV. An investigation determined no abuse occurred by AP2.
