- According to the documentation, the facility failed to implement appropriate fall interventions for the Alleged Victim’s (AV) fall history. From approximately July 30, 2024, through September 26, 2024, the AV experienced approximately 9 falls. There was no documented evidence the facility implemented progressive interventions to prevent future falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse
