Findings
According to the documentation, the Alleged Perpetrator (AP2) failed to provide a safe medication administration system. The failure resulted in the Alleged Victim (AV) to receive the wrong medication due to the AP2 not following facility medication protocol, which is a violation of resident rights and is considered abuse. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
