- The facility failed to implement interventions and appropriately care plan regarding Alleged Victim’s (AV) known fall risk. Between April 7, 2023, and October 10, 2023, AV had approximately eight (8) documented falls. On or about October 17, 2023, AV fell and was transported to the hospital and diagnosed with a fractured ankle, a concussion, and a laceration of the head. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
