Findings
The facility failed to provide appropriate supervision and follow Witness 2's (W2) care plan to ensure s/he is not left alone with other residents due to aggressive behavior and prior altercations. The failure resulted in engaging in a physical altercation with the Alleged Victim, causing unreasonable discomfort and bruising, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
