Findings
The Alleged Victim (AV) receives his/her medications from facility staff. On or about October 21, 2023, Alleged Perpetrator #3 (AP3) was dispensing medications and failed to follow the care plan to watch AV take his/her medications, which was dissolved in liquid, allowing AV's medication to stay unattended overnight, placing AV at risk of harm for not taking his/her medication and other residents that could have drank AV's medication from the glass. AP3's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
