Findings
The Alleged Victim (AV) and Witness #1 (W1) are known to engage in altercations. On or about October 14 2023, AV was pushed by W1 and fell to the floor. ON or about October 15, 2023, staff heard screaming and went to the family room where they saw W1 hitting AV in the head with his/her fists. AV fell to the floor and was sent out to the hospital for evaluation. The facility's failure to properly care plan to reduced the risk of altercations between residents is a violation of resident rights, is considered neglect of care and constitutes abuse.
