Findings
The Alleged Victim (AV) and Witness #1 (W1) are known to engage in altercations with each other. On or about October 17, 2023, W1 hit AV in the side and pulled his/her blankets off trying to get AV out of bed. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
