Findings
On or about October 26, 2023, the Alleged Victim (AV) was being picked up from a medical appointment by Alleged Perpetrator #2 (AP2). While AP2 was assisting AV into the van for transportation, AV's wheelchair fell backwards and AV hit his/her head on the ground. AV was sent to the hospital and diagnosed with closed head injury and a strain to the lumbar region. Days after the fall, AV suffers from headaches, pain in his/her lower back and an intense fear of leaving the facility. AP2 was not properly trained by the facility to transport residents, according to documentation. AP2's actions were investigated and determined to be unsubstantiated for abuse. The facility's failure to ensure proper training of staff in regards to loading and unloading residents into the facility vehicles is a violation of resident rights, is considered neglect of care and constitutes abuse.
