Findings
The Alleged Victim (AV) has had several falls. On or about August 15, 2023, AV fell and received a bruise, knee pain and side pain. AV fell again on September 8, 2023, and hit his/her head. The facility failed to implement meaningful interventions to prevent falls, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
