Findings
The facility failed to intervene when the Alleged Victim ‘s (AV) experienced a short-term change of condition. AV is care planned for a medical condition that appears evident when AV displays increased confusion and odd behavior. According to an investigation and documentation, between October 18,2023, to October 27, 2023, AV experienced significant changes in behaviors. On or about October 29, 2023, AV experienced a fall that resulted in discoloration to AV’s face and was transported to the hospital where his/she was diagnosed with a UTI. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
