Findings
The facility failed to follow Witness 1's (W1) care plan to provide 1:1 supervision according to his/her behaviors and altercations. The failure resulted in an unwitnessed altercation, staff heard yelling and saw W1 pulling AV's hair, causing AV unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
