Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about November 4, 2023, AV was given medication that belonged to a different resident. Witness knowledge and documentary evidence indicated hat on the evening of November 4, 2023, Alleged Perpetrator 2 (AP2) was distracted during the evening med pass and gave AV other resident’s medications. There was a total of six (6) medication given by AP2 to AV. AV failed to provide a safe medication system to AV, which is neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system for AV, which is a violation of Oregon Administrative Rules.
