Findings
According to the documentation, the facility failed to properly care plan for Witness 4’s (W4) known verbally abusive towards other residents. On or about November 5, 2023, W4 was verbally abusive towards the Alleged Victim (AV) resulting in the AV to targeted and uncomfortable around W4, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
