Findings
The Alleged Victim (AV) relies on the facility for care and has a history of falls. According to an investigation, on or about November 12, 2023, AV had an unwitnessed fall, resulting in a leg injury. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
