- Witness #1 (W1) has a history of resident to resident altercations. It was determined that W1 exhibits agitation by undressing him/herself. On or about November 7, 2023, W1 engaged in an altercation with the Alleged Victim (AV) where W1 hit AV. W1 was pulling his/her pants down prior to the altercation, and staff failed to redirect W1 away from AV to avoid the altercation. The facility's failure to follow interventions in place is a violation of resident rights, is considered neglect of care and constitutes abuse.
