Findings
On or about December 8, 2023, it was noted that the Alleged Victim's (AV) toenails were thickened and yellow, possibly indicating a fungal infection. AV's toenails were overgrown to the point of causing pain when wearing shoes. Staff are to complete nail care for AV, however, it is not known how long it has been since any toenail care was performed on AV. The facility's failure to provide nail care to AV caused unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
