Findings
On or about September 22, 2023, the Alleged Victim (AV) punched Witness 1 (W1) in the face and then fell backwards injuring their thumbnail. AV expressed pain when cleaning and bandaging the wound. The facility failed to provide a safe environment and follow the plan of care resulting in physical harm to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
