Findings
The facility failed to follow the Alleged Victim's care plan to provide assistance with showers. The failure resulted in AV receiving approximately 5 showers in the months of October and November of 2023, causing unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
