- Alleged Perpetrator 2 (AP2) neglected Alleged Victim (AV) by failing to provide appropriate care to AV which resulted in unreasonable discomfort and loss of personal dignity. On or about November 27, 2023, AV's care tasks were not completed, which resulted in AV remaining in his/her wheelchair throughout the night and was found by morning staff in his/her wheelchair with a soiled brief. Based on facility documentation and interviews, AP2 did not perform care tasks per AV's Service Plan and falsified documentation stating that he/she did complete the tasks of a safety check and dressing in the evening. During the morning checks, AV was found in his/her wheelchair crying and the lights off. AV was found with a soaked brief and AV's bed was not made. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a service for AV which is a violation of Oregon Administrative Rules. Alleged Perpetrator 3 (AP3) allegedly neglected AV. An investigation determined no abuse occurred by AP3.
