Findings
The facility failed to implement interventions or appropriately care plan for Witness #2’s (W2) known increasing behaviors. On or about November 25, 2023, W2 cursed at and hit the Alleged Victim (AV) in the arm/shoulder, causing unreasonable discomfort. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
