Findings
According to the documentation, the facility failed to follow the care plan for the Alleged Victim (AV) by ensuring the AV’s mobility device was available. The failure resulted in the AV attempting to ambulate without an assistive device and suffered a fall sustaining a black eye and skin tear under their eye, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
