Findings
On or about December 08, 2023, Alleged Perpetrator 2 (AP2) failed to follow The Alleged Victim (AV) care plan. AV needs a two-person transfer along with bed positioning to safely. AP2 assisted AV without communication, resulted in AV falling over h/her bed and hitting h/h head suffering pain, and unreasonable discomfort. AP2's actions is a violation of resident rights, is considered neglect of care and constitute abuse.
