Findings
The facility failed to implement interventions, appropriately care plan and monitor Witness 1 (W1) according to his/her aggression and prior resident altercations. The failure resulted in W1 punching the Alleged Victim in the arm, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
